Lease Termination Request

*Please note:
- This form must be submitted 60 or 30 days in advance of vacating your unit as as outlined in paragraph 1 of the lease.
- Your unit must be vacated by 9:00 a.m. on the date of your lease termination. 
- Your unit should be left in the same condition as when you found it in order to receive the refundable portion of your deposit ($275).
- This form is provided as a convenience to our clients, but is susceptible to normal internet delivery failures. You should receive an automated receipt from our system, but if you do not receive confirmation of receipt of this notice from your STLCH representative within 24 hours, please contact your representative.
